There is a waiting list at Tavistock Community Health.

Doctors and health care staff at the centre recognize the value of building and maintaining a positive working relationship between the patient and the health care professional. Patients have a strong interest in ensuring that the relationship is marked by openness, honesty and respect. As such, each patient and his/her doctor must determine if they are a "good match" and if they can build a long-term health care relationship.

Patients at the centre are asked to register with the Tavistock Family Health Network. During the registration process a patient has to identify one physician as his or her principal physician. In reality the patient will be a patient of the group practice. If a patient needs immediate care during centre hours, he or she will be treated by the doctor on call.

Hours of Service

The Health Centre is open:
8:30am - 5pm: Monday to Thursday
8:30am - 8pm: Monday to Thursday (if evening Clinic in Tavistock)
8:30am - 5pm: Friday
9:00am - 12 noon - Saturday (if Clinic in Tavistock)

Patients registered at the centre are able to use the toll-free 1-866-553-7205 Telephone Health Advisory Service number. This is a free service available weekdays from 5 pm to 9 am and 24 hours on weekends and official holidays. When you call 1-866-553-7205 you speak with an Ontario Registered Nurse who provides you with medical advice on how to best treat your health care problem. The nurse advises the Tavistock Family Health Network so that appropriate follow-up action can be taken. If the Telehealth Nurse has a concern regarding a patient who has called, she may consult directly with the Physician on call for the Star Family Health Team

Making Appointments

To book an appointment, call 655-2322.

Appointments can be virtually immediate or they can be scheduled several weeks or months in the future. It all depends upon the need.

If you have an immediate need … like a sore throat or a cut finger or any type of medical problem that you believe requires immediate attention … you can be treated by the doctor assigned to be “on call”. On call appointments are scheduled for the same day or the next day.

If you are calling to book a routine physical exam or some similar routine procedure you could be scheduled some time into the future.

If you are calling to book a return appointment as requested by your health care provider, please provide this information to the Receptionist when you call.

When you call for an appointment, please have handy any schedules or work shifts or other commitments that may conflict with an appointment. This information will assist both you and the Receptionist in finding a mutually suitable date.

Cancelling Appointments

If you are unable to keep an appointment, please call during office hours to officially cancel it. The number to call for cancellations is 655-2322. Whenever possible, please provide at least 24 hours notice.

If you do not call to officially cancel an appointment that you cannot attend, you could be charged a “missed appointment” fee. Please see the section entitled “Our Patients – Fees Charged” in this website for more details about fees.

Prescriptions

Prescriptions for medication are written by your doctor for a specific period of time. The prescription may be “once only” or may allow for refills. After this time, your doctor may require you to make another appointment before writing another prescription.

If you are calling about a prescription, please have the prescription label handy to check the exact medication name, when prescribed and the number of refills prescribed.

Emergency Service

Emergency care … such as heart failure or stroke or seizure … is best treated at a hospital where specialized equipment and personnel are readily available.

If you require emergency service, please call 911 for ambulance service or report to the Emergency Room at your nearest hospital.

The hospital will notify your family doctor if you are admitted.

Fees Charged

Certain services not covered by the Ministry of Health fee structure and provided by the Tavistock Family Health Network are chargeable.

The following list represents some of the services that can be offered at the Tavistock Health Centre as well as their current fee (based on the Ontario Medical Association Schedule of Fees). Payment for service is due on the day of treatment unless prior financial arrangements have been made with the centre. Fees are subject to change without notice.
