Overview

Tavistock Community Health Inc. (TCHI) is a non-profit, community-owned and supported health care organization serving the greater Tavistock area.

Centre in not-winter.

Four family physicians, (with room to accommodate up to six physicians) operating under the auspices of the Tavistock Family Health Network, nurses and administrative staff, work at the Tavistock Health Centre, a facility owned and managed by TCHI.

Approximately 7,000 patients use the Health Centre's services. In addition, the physicians provide in-house care for residents at Tavistock's 3 long-term care facilities. Patients requiring hospital care are generally referred to Stratford General Hospital where the Tavistock family physicians have hospital privileges.

Tavistock is recognized and respected as a teaching facility for family physicians, sponsoring field placements/training for medical students and residents from the University of Western Ontario, McMaster University and the South Western Ontario Rural Regional Medicine Program (SWORRM).

Tavistock is situated at the crossroads of 3 counties in the heart of southwestern Ontario … Oxford, Perth, and Waterloo. These 3 counties have a combined population base of approximately 550,000.

Tavistock combines the best of village life with easy access to major centres … 17 km to Stratford, 35 km to Kitchener-Waterloo, 60 km to London, and 140 km to Toronto.

The Tavistock Health Centre is located at 80 Maria Street in Tavistock. At the 5-way intersection in Tavistock (traffic light) turn south onto Maria Street (one way) towards the Arena and Memorial Hall. The centre is at the end of Maria Street, on the right-hand side across from Tavistock Public School.

Hours of Service

The Health Centre is open:
8:30am - 5pm: Monday to Thursday
8:30am - 8pm: Monday to Thursday (if evening Clinic in Tavistock)
8:30am - 5pm: Friday
9:00am - 12 noon - Saturday (if Clinic in Tavistock)

Patients registered at the centre are able to use the toll-free 1-866-553-7205 Telephone Health Advisory Service number. This is a free service available weekdays from 5 pm to 9 am and 24 hours on weekends and official holidays. When you call 1-866-553-7205 you speak with an Ontario Registered Nurse who provides you with medical advice on how to best treat your health care problem. The nurse advises the Tavistock Family Health Network so that appropriate follow-up action can be taken. If the Telehealth Nurse has a concern regarding a patient who has called, she may consult directly with the Physician on call for the Star Family Health Team

Facilities

The Tavistock Health Centre consists of a 6,000 square foot building with 7 physician offices, 14 examination rooms, administrative staff offices and medical records storage.

Hospitalized patients are generally referred to and treated at Stratford General Hospital which is located 17 km from Tavistock. Stratford General Hospital is the regional referral community hospital in the Huron Perth Healthcare Alliance. Tavistock's family physicians provide in-house care to patients at 3 long-term care facilities … Bonnie Brae Health Care Center, Maples Home For Seniors, and peopleCare Health Centre … all located in Tavistock.

Programs and Services

The family physicians provide care in the areas of adult medicine, geriatrics, well-baby checks and pediatrics, and women's health. When and as required, they provide referrals to a wide network of specialists.

Flu shot clinics are arranged annually for patients of the Health Centre.

Health Care Programmes with a specific focus on Preventive Medicine are offered routinely.

A Skin Clinic is run monthly by Dr. Card, one of the family physicians.

Dr. Michael Gillett, an internist at Stratford General Hospital, uses the Tavistock Health Centre as a satellite office to serve his patients in the Tavistock area. Appointments with Dr. Michael Gillett are arranged by referrals from the Tavistock Physicians.

The STAR Family Health Team is comprised of the following three sites:

Allied Health Professionals including Mental Health Worker, Pharmacist, Nurse Practitioner, Psychologist and Dietician.

Board of Directors

Tavistock Community Health Inc. (TCHI) is managed by a Board of Directors. The Directors are volunteers from the community who serve for 3-year terms.

Membership and Donations

Individuals interested in sustaining health care in the community are encouraged to become a member of TCHI. Membership is currently set at $5 per annum. Members have voting privileges at Annual General Meetings.

Maintaining a health care facility is a costly undertaking. All forms of monetary contributions (e.g. donations, bequests, in memorium) are sincerely appreciated. Monetary contributions to TCHI are used for a variety of purposes ranging from recruiting new doctors to purchasing needed medical equipment to ensuring that the building structure meets required standards.

TCHI is a registered charity. Receipts for income tax purposes are provided.
